Resumen
Determinar la sensibilidad y la especificidad del índice de pulsatilidad a través del ultrasonido doppler para el diagnostico de pielonefritis en niños menores de 6 años con primer episodio de infección de vías urinarias febril.
DISEÑO DEL ESTUDIO: Validación de prueba diagnóstica dentro de una corte de pacientes menores de 6 años con sospecha clínica de primer episodio de infección urinaria alta febril (pielonefritis) hospitalizados en el Hospital Militar Central, Bogotá-Colombia entre el 15 de abril y el 30 de septiembre dl 2013.
RESULTADOS: Del total de unidades renales estudiadas, 14 fueron diagnosticadas con pielonefritis por gammagrafía con DMSA, en este grupo es más frecuente el hallazgo ecográfico de anormalidad sugestiva de pielonefritis y los índices de resistencia y pulsatilidad son mayores en los sujetos con infección, en todas las edades. El promedio del índice de resistencia en sujetos sin pielonefritis fue de 0,64 (IC95 0,622 – 0,67) en la arteria renal derecha y 0,64 (IC95 0,62 – 0,67) en la izquierda. En sujetos con pielonefritis el incide de resistencia aumenta a un promedio de 0,72 (IC95 0,69 – 0,76) en la arteria renal derecha y a 0,73 (IC95 0,70 – 0,76) en la izquierda. El índice de pulsatilidad derecho en estos pacientes tuvo un promedio de 1,63 (IC95 1,45 – 1,81) y el izquierdo uno de 1,65 (IC95 1,39 – 1,91). La validez del índice de pulsatilidad en términos de sensibilidad, especificidad, VPP y VPN fue de 100%, 79.1%, 58.3% y 100% para la arteria renal derecha y de 100%, 95.8%, 87.5%, 100% para la arteria renal izquierda, respectivamente. El índice de correlación Kappa de acuerdo a lo anterior fue mejor para el lado izquierdo que derecho: 0,63 vs 0,91. Se evidencio un menor rendimiento en el índice de resistencia derecho, con una sensibilidad de 28% (IC 0 – 69,18) y una especificidad del 95% (IC95 85,76 – 100) contra una sensibilidad del 88% (IC95% 52,65 – 100) para el índice de resistencia izquierdo.
CONCLUSIONES: El índice de pulsatilidad tiene una buena sensibilidad y especificidad para el diagnostico de pielonefritis cuando se compara con la gammagrafía DMSA y su rendimiento global es bueno y estadísticamente significativo, independiente de la lateralidad, siendo similar al que se logra con el índice de resistencia y superior a la ecografía bidimensional. Se requieren estudios con mayor tamaño de muestra para corroborar los hallazgos encontrados en nuestro estudio.
To determine the sensitivity and specificity of the pulsatility index through doppler ultrasound for the diagnosis of pyelonephritis in children under 6 years with a first episode of febrile urinary tract infection .
STUDY DESIGN : Validation Diagnostic Test in a court under 6 patients with clinically suspected first episode of febrile upper urinary tract infection (pyelonephritis ) hospitalized at the Hospital Militar Central , Bogotá , Colombia from 15 April to 30 dl September 2013.
RESULTS : Of the studied renal units , 14 were diagnosed with pyelonephritis DMSA scan , this group is more common ultrasound finding suggestive abnormality of pyelonephritis and rates of resistance and pulsatility are higher in patients with infection, all ages. The average resistance index in subjects without pyelonephritis was 0.64 (IC95 0.622 to 0.67 ) in the right renal artery and 0.64 ( CI95 0.62 to 0.67 ) on the left . In subjects with pyelonephritis the impact resistance increases to an average of 0.72 ( IC95 0.69 to 0.76 ) in the right renal artery and 0.73 ( CI95 0.70 to 0.76 ) on the left . The right pulsatility index in these patients had an average of 1.63 ( IC95 1.45 to 1.81 ) and left one of 1.65 ( IC95 1.39 to 1.91 ) . Validity of pulsatility index in terms of sensitivity , specificity , PPV and NPV were 100 % , 79.1 % , 58.3 % and 100% for the right renal artery and 100 % , 95.8 % , 87.5 % , 100% for the artery left renal respectively. The Kappa correlation index according to the above was best for the left side right : 0.63 vs 0.91. Lower performance in the right resistance index was evidenced , with a sensitivity of 28 % (CI 0 to 69.18 ) and a specificity of 95 % ( IC95 85.76-100 ) with a sensitivity of 88 % (95% CI 52 , 65 - 100) to the left index of resistance .
CONCLUSIONS : The pulsatility index has a good sensitivity and specificity for the diagnosis of pyelonephritis when compared with DMSA scintigraphy and its overall performance is good and statistically significant , regardless of laterality , being similar to that achieved with the resistance index and superior to the two-dimensional ultrasound. Studies with larger sample size are needed to corroborate the findings in our study.
