Resumen
Introducción: Las opciones de tratamiento quirúrgico en artrosis patelofemoral grado IV secundaria a malalineamiento patelofemoral son limitadas. Los procedimientos quirúrgicos más utilizados son el realineamiento distal de la rótula mediante los diferentes tipos de osteotomías del tubérculo tibial con resultados variables según lo reportado en la literatura. Objetivo: Evaluar la funcionalidad y el nivel de dolor en una cohorte de pacientes con artrosis patelofemoral grado IV secundaria a malalineamiento patelofemoral, tratados mediante osteotomía Fulkerson más injerto óseo tricortical, entre 2001 y 2011.
Materiales y métodos: Se realizó un estudio de cohorte retrospectiva única de pacientes con artrosis patelofemoral grado IV secundaria a malalineamiento patelofemoral que fueron tratados mediante osteotomía Fulkerson más injerto óseo tricortical entre 2001 y 2011. Los resultados fueron evaluados mediante la aplicación de la escala funcional de Lysholm modificada por Fulkerson y la escala visual análoga para el dolor (EVA). El seguimiento promedio fue de 44,0 ± 26,8 meses (mediana = 34,3 meses) con un rango entre 2 y 11,9 años.
Resultados: Un total de 38 pacientes (45 rodillas, 7 bilaterales) fueron tratados con la técnica descrita. El más frecuente fue el género femenino con 92,1% (n=35) y la edad promedio fue de 44,7 ± 10,9 años (rango entre 23 y 62 años.) El 100%de las rodillas evaluadas presentó mejoría en la EVA. Los resultados de la escala funcionales reportan un 93,3% entre excelentes y buenos resultados. No se presentó deterioro de la mejoría durante el tiempo de seguimiento. No se presentaron complicaciones mayores.
Conclusión: La osteotomía de realineamiento distal tipo Fulkerson más injerto óseo tricortical y espongialización mostró un adecuado resultado funcional y disminución del dolor a mediano plazo para el manejo de la artrosis patelofemoral grado IV.

Background: The surgical treatment options in severe patellofemoral arthritis secondary to patellofemoral malalignment are limited. The majority of the surgical procedures include distal patellar realignment through different types of tibial tuberosity osteotomies, current evidence reports different results. Aim: evaluate the functionality and the level of pain in a cohort of patients with patellofemoral arthritis secondary to patellofemoral malalignment treated by Fulkerson osteotomy plus tricortical bone graft between 2001 y 2011.
Methods: We conducted a retrospective cohort study of patients with severe patellofemoral arthritis secondary to patellofemoral malalignment treated by Fulkerson osteotomy plus tricortical bone graft between 2001 and 2011. The results were evaluated using the Lysholm scale modified by Fulkerson and the VAS score. A mean follow-up of44.0± 26.8 months, range 2-11.9 years.
Results:A total of 38patients (45 knees, 7 bilateral) were treated using the technique described, 35 women (92.1%). An average age of 44.7±10.9 years (range between 23 and 62 years). All of the patients showed an improvement in the VAS score. The results of the functional scales were 93.3% excellent and good results. During follow up we do not report deterioration. We do not report major complications.
Conclusions: Fulkerson osteotomy plus tricortical bone graft is an adequate treatment in functionality and decreased pain severity in the short term in patients with patellofemoral arthritis secondary to patellofemoral malalignment.
