dc.description.abstract | Antecedentes La colangiopancreatografía retrógrada endoscópica es una terapia apropiada en el manejo de la coledocolitiasis, sin embargo existen controversias sobre su seguridad en el paciente adulto mayor. Objetivo Determinar si existe un mayor riesgo de complicaciones con la realización de la colangiopancreatografía retrógrada endoscópica (CPRE) terapéutica para la Coledocolitiasis en pacientes los mayores de 70 años de edad en el Hospital Universitario Clínica San Rafael. Metodología Se realizó un estudio de corte analítico que incluyó a los pacientes que ingresaron al servicio de urgencias del Hospital Universitario Clínica San Rafael durante un año (2010-2011) con diagnóstico de coledocolitiasis de alta probabilidad o confirmada. Se realizó una estimación del tamaño muestral. Se realizó el análisis de la información teniendo en cuenta dos grupos de pacientes (mayores y menores de 70 años). Para la comparación de variables numéricas se empleó el test T de Student para muestras independiente o la prueba de Mann Withney previa determinación de la distribución normal. Resultados Se incluyeron 171 pacientes. La presencia de comorbilidades es más frecuente en los mayores de 70 años siendo las más frecuentes la diabetes y la hipertensión arterial. Los niveles de bilirrubina directa, TGO y TGP fueron mayores en pacientes de más de 70 años. La CPRE mostró ser eficaz independientemente de la edad. Se presentaron complicaciones relacionadas con el procedimiento siendo mas frecuente la presencia de Pancreatitis Post CPRE, sin embargo estas no están relacionadas con la edad de los pacientes. Conclusión La CPRE es eficaz y segura en pacientes con coledocolitiasis. La edad mayor de 70 años no incrementa la mortalidad relacionada con el procedimiento. Se requieren estudios prospectivos para confirmar estos datos. | spa |

dc.description.abstractenglish | BACKGROUND Endoscopic retrograde cholangiopancreatography is an appropriate therapy for the management of choledocholithiasis. However, there is controversy about its safety in elderly patients. Objective: To determine whether there is an increased risk of complications when performing therapeutic endoscopic retrograde cholangiopancreatography (ERCP ) for choledocholithiasis in patients older than 70 years old, at the Hospital Universitario Clínica San Rafael . Materials and methods An Analytical cut study was made on all patients admitted to the emergency room and diagnosed with high probability choledocholithiasis, at the Hospital Universitario Clínica San Rafael for a year (2010-2011). A sample size estimation was performed. Information analysis considering two groups of patients (older and younger than 70 years) was performed. To compare numerical variables a Student 's t test for independent samples and the Mann Whitney prior determination of the normal distribution were used. Results 171 patients were analysed. The presence of comorbidities was more common in people over 70 years, the most frequent being diabetes and hypertension. The direct bilirubin, AST and ALT were higher in patients over 70 years. ERCP showed to be effective regardless of age. There were procedure-related complications, most of them being the presence of post ERCP Pancreatitis. However, these were not found to be related to the patient’s age . Conclusion ERCP is effective and safe in patients with choledocholithiasis. Mortality associated with the procedure does not increase in patents aged 70 and over. Prospective studies are needed to confirm these data. | eng |
