Resumen
Los estudiantes de medicina durante su año de internado, se ven enfrentados directamente al paciente y sus familiares, quienes a su vez buscan en el interno respuesta a sus problemas de salud. Durante su carrera no han tenido la posibilidad de desarrollar conductas autonómicas que guíen su protagonismo en la relación médico-paciente, lo cual, de cara al primer año como profesional, lo coloca inmediatamente ante situaciones no experimentadas durante su carrera, que pueden llegar a afectar su relación con el paciente y su familia desde el punto de vista de la comunicación asertiva sobre su estado de salud y el planteamiento de la terapéutica a seguir.
El presente ensayo se propone determinar, desde el punto de vista del médico profesor, el nivel de autonomía que el interno puede llegar a desarrollar durante su último año de carrera.
Se realizó revisión documental sobre la autonomía en la carrera de medicina, así como de la relación médico - paciente. Se diseñó y aplicó una encuesta dirigida a docentes, en la cual, se indagó sobre la autonomía y su implementación en el quehacer diario del interno, posteriormente se consolidó la información para realizar el análisis y obtener las conclusiones pertinentes.
Existe cierto nivel de autonomía con el cuál el médico interno inicia su sexto año de formación, el cual debe ser explotado por el mismo y por el medico docente para lograr desarrollar sus propios perfiles así como, al mismo tiempo, salvaguardar los principio éticos que rigen la relación médico – paciente. Las universidades deben redefinir, de la mano de las instituciones de salud, las competencias de las que goza el médico interno en su quehacer diario respetando siempre los principios éticos que protegen al paciente, y contextualizado la posición del médico interno en el contexto del sistema de salud en el que dicha relación se desenvuelve.
Medical students during their year of internship, are directly confronted with the patient and their relatives, who in turn seek in the internal response to their health problems. During his career they have not had the possibility of developing autonomous behaviors that guide his role in the doctor-patient relationship, which, facing the first year as a professional, immediately places him before situations not experienced during his career, which may affect his relationship with the patient and his family from the point of view of assertive communication about his state of health and the approach of the therapy to follow.
The present essay intends to determine, from the point of view of the medical professor, the level of autonomy that the intern can develop during his last year of career.
A documentary review was made about the autonomy in the medical career, as well as the doctor - patient relationship. A survey was designed and applied to teachers, in which, the autonomy and its implementation in the daily work of the inmate was investigated, later the information was consolidated to carry out the analysis and obtain the pertinent conclusions.
There is a certain level of autonomy with which the internal doctor begins his sixth year of training, which must be exploited by the same and by the teaching physician to develop their own profiles as well as, at the same time, safeguard the ethical principles that govern the doctor-patient relationship. Universities must redefine, from the hand of the health institutions, the competences enjoyed by the internal doctor in their daily work, always respecting the ethical principles that protect the patient, and contextualizing the position of the internal doctor in the context of the system of health in which this relationship develops.
