dc.description.abstract | Introducción: La Enfermedad Inflamatoria Intestinal (EII) comprende colitis ulcerativa (CU) y enfermedad Crohn (EC), son patologías inflamatorias crónicas del tracto gastrointestinal, con baja prevalencia, que además presentan una afectación sistémica, con manifestaciones extraintestinales frecuentes. Su curso clínico presenta múltiples recaídas y aumento en la frecuencia de aparición a nivel mundial. Materiales y Métodos: Serie de casos a que evalúa las características epidemiológicas de la EII en la población del subsistema de salud de las Fuerzas Militares en Colombia en el periodo comprendido entre 2012-2017. Datos obtenidos de revisión de historias clínicas e informes endoscópicos almacenados en sistema Medicap y Dinámica Gerencial. Con SPSS 20 se obtuvo medidas de tendencia central y dispersión para variables continuas y frecuencias. Resultados: La población total posterior a confirmación histológica fueron 140 pacientes (78,6% con CU, 15,7% con EC y 5,7% con CI). Antecedente familiares de EII en el 7,3% y antecedente de enfermedad autoinmune hasta el 9.1%. Las manifestaciones extraintestinales se presentaron en 51,8% de los pacientes con CU, 72,7% en EC y 37,5% en CI. La clasificación endoscópica en CU mostro una puntuación de Montreal en E3 en el 41,8% y S3 en el 41,8%. En EC la edad fue A2 en el 45,5%, localización 52,4% y el 57,1% fueron B2. Conclusiones: En nuestra población, la EEI muestra distribución de género y edades similares a poblaciones mundiales. CU tiene un fenotipo en el cual se acentúa una gran extensión (Montreal E3), escasa sintomatología intestinal, pocos brotes, y compromiso extraintestinal frecuente, asociándose este fenotipo a riesgo de cáncer colorectal aumentado. EC se encuentra diagnóstico tardío, requiriendo manejo quirúrgico en la mitad de los pacientes al momento del diagnóstico, con fenotipo estenosante de ubicación ileocecal. Sugiriendo estos datos un tamizaje que incluya la canulación del íleon en las colonoscopias diagnósticas y de seguimiento. | spa |

dc.description.abstractenglish | Introduction: Inflammatory Bowel Disease (IBD) includes ulcerative colitis (UC) and Crohn's disease (CD), are chronic inflammatory pathologies of the gastrointestinal tract, with low prevalence, which also have a systemic involvement, with frequent extraintestinal manifestations. Its clinical course has multiple relapses and an increase in the frequency of occurrence worldwide. Materials and Methods: Series of cases that assess the epidemiological characteristics of IBD in the population of the health subsystem of the Military Forces in Colombia in the period between 2012-2017. Data obtained from review of medical records and endoscopic reports stored in the Medicap system and Management Dynamics. With SPSS 20, measures of central tendency and dispersion for continuous variables and frequencies were obtained. Results: The total population after histological confirmation was 140 patients (78.6% with UC, 15.7% with CD and 5.7% with IC). Family history of IBD in 7.3% and history of autoimmune disease up to 9.1%. Extraintestinal manifestations occurred in 51.8% of patients with UC, 72.7% in CD and 37.5% in IC. The endoscopic classification in CU showed a Montreal score in E3 in 41.8% and S3 in 41.8%. In EC the age was A2 in 45.5%, location 52.4% and 57.1% were B2. Conclusions: In our population, the IEE shows gender distribution and ages similar to world populations. CU has a phenotype in which a large extension is accentuated (Montreal E3), poor intestinal symptoms, few outbreaks, and frequent extraintestinal involvement, associating this phenotype with increased colorectal cancer risk. EC is late diagnosis, requiring surgical management in half of the patients at the time of diagnosis, with stenosant phenotype of ileocecal location. Suggesting this data a screening that includes cannulation of the ileum in diagnostic and follow-up colonoscopies. | eng |
