dc.description.abstract | La hipoglicemia neonatal es frecuente. Puede presentarse de forma transitoria o persistente. Existen diferentes puntos de corte de valor de glucosa en sangre para definir hipoglicemia (usualmente menores de 47 mg/dl), que varían según la referencia bibliográfica que se tome . Así mismo muchos estudios evidencian factores de riesgo tanto maternos como neonatales para desarrollar esta enfermedad y se ha demostrado que el retraso en el tratamiento de la hipoglicemia tiene alteraciones en el neurodesarrollo. El riesgo de padecer hipoglicemia teniendo factores de riesgo varía desde un 14.7% y 83.2%; respecto a los pacientes en los cuales no se presentan dichos factores que data de un 5% Objetivo: determinar los factores de riesgo asociados a hipoglicemia neonatal transitoria de recién nacidos a término; que no cumplieran factores de riesgo para hipoglicemia determinados por la guía de la Academia Americana de Pediatría, en el Hospital Universitario Clínica San Rafael en el periodo comprendido entre Enero 2015 a Enero de 2019. Metodología: se presenta un estudio observacional analítico, retrospectivo de tipo casos y controles, desde Enero de 2015 a Enero de 2019. Se realiza cálculo de muestra y se obtiene 35 casos y 35 controles mediante el software estadístico Real Statistics 6.9 de Enero de 2020 . Resultados: se recogieron datos del periodo Enero de 2015 a Enero de 2019, recolectándose un total de 70 pacientes que cumplían los criterios de inclusión, 35 casos y 35 controles. La mayor proporción de pacientes en el estudio fue de sexo femenino (61.43%), 20 casos y 23 controles, con OR para sexo masculino de 1.4. La edad gestacional, las medidas antropométricas del recién nacido, fueron similares en ambos grupos. El promedio de glucosa de ingreso de los casos fue de 40.8 mg/dl. La edad de las maternas analizadas con diferencia de dos años no mostró asociación alguna con la patología en estudio, el promedio de gestación para los casos fue de uno y de los controles dos. El sobrepeso materno mostró un OR de 1.2. El OR para cesárea fue de 2.5 en nuestra cohorte de pacientes, practicándose ésta en 15 (42.8%) casos. La hipertensión inducida por el embarazo en 4 casos y 1 control, mostrando un OR de 4.38 para hipoglicemia. Mostró asociación significativa: APGAR al minuto de 5 en 3 (8.57%) de los casos; el primer aporte energético recibido del recién nacido de los cuales 25 (71.4%) de los casos recibieron lactancia materna; la TTRN en 6 (17.14%) casos y la policitemia en 10 (28.57%) de los casos, como variables en el recién nacido y tiempo de apego como variable materna que mostró asociación en 7 (20%) casos que no la recibieron. Conclusiones: nuestro estudio aporta a la literatura actual, el hecho de incluir población a término considerada “sana” que desarrolla hipoglicemia neonatal transitoria teniendo como variables asociadas el APGAR al minuto 5, el primer aporte energético recibido, la TTRN, la policitemia y el apego. Variables que en su mayoría influyen en el metabolismo de utilización de la glucosa y que posteriomente sus efectos se pueden ver a largo plazo en las habilidades motoras y neurodesarrollo de la población afectada por ésta patología. Es un llamado a prestar atención a este grupo de recién nacidos, que bien presentan con menor incidencia hipoglicemia, sí necesitan de nuestro acompañamiento y seguimiento dado que se ve influenciada como hemos visto por distintos factores. | spa |

dc.description.abstractenglish | The neonatal hypoglycemia is a frequent pathology. It can show as a transient or persistent presentation. There are many different cut value points of blood glucose to define hypoglycemia (usually less than 47 mg/dl) depending on the reference taken . Likewise there are many different studies showing maternal risk factors like newborn factors to develop this entity and it has showed that the delay in the treatment has neurodevelopment outcomes. (5)(9) The risk of hypoglycemia having risk factors varies from 14.7% to 83.2%; with respect to patients in wohom these factors are not present, which dates from 5%. Objective: to determine the risk factors associate with transient neonatal hypoglycemia in term infants; that didn´t meet risk factors for hypoglycemia determined by the guide of the American Academy of Pediatrics, at the Hospital Universitario Clínica San Rafael in the period from january 2015 to January 2019.Methodology: An observational, analytic, retrospective case-control study is presented, from January 2015 to January 2109. Sample calculation is performed and 35 cases and 35 controls are obtained using the Real Statistics software 6.9 of January 2020. Results: Data were collected from the period January 2015 to January 2019, collecting a total of 70 patients who met the inclusion criteria, 35 cases and 35 controls. The highest proprotion of patients in the study was female (61.43%), 20 cases and 23 controls, with a male OR of 1.4. Gestational age, anthropometric measurements of the newborn, were similiar in both groups. The average glucose admission of the cases was 40.8 mg/dL. The age of the maternal analyzed with a difference of two years didn´t show any association with the pathology of study, the average gestation for the cases was one and the controls two. Maternal overweight showed an OR of 1.2. The OR for caesarean section was 2.5 in our cohort of patients, being performed in 15 (42.8%) cases. Pregnancy-induced hypertension in 4 cases and 1 control, showing an OR of 4.38 for hypoglycemia. It showed significant association: APGAR score at the first minute of 5 in 3(8.57%) of the cases; the first energy intake received of the newborn, of which 25 (71.4%) of the cases received breastfeeding; NRT in 6 (17.14%) cases and polycythemia in 10 (28.57%) of the cases, as variables in the neonate and skin to skin contact as a maternal variable that showed association in 7 (20%) cases that didn´t receive it. nConclusions: our study contributes to the current literature in the fact of including term population considered “healthy” that developes transient neonatal hypoglycemia, having as associate variables the APGAR at minute of 5, the first energy intake receidev, the NRT, polycythemia and skin to skin contact. Variables that mostly influence metabolic glucose utilization and later its effects can be seen in long term in the motor skills and neurodevelopment of the population affected by this pathology. It is a call top ay attention to this group of newborns, who well present with a lower incidence of hypoglycemia they need our surveillance and follow up since that it is influenced as we have seen by different factors. | eng |
