Resumen
La incidencia de hemotórax retenido es del 2- 20% (4,9) en pacientes que presentan hemoneumotórax posterior a un traumatismo en estadísticas fuera de Colombia, sin embargo no hay estudios suficientes para comparar dicha incidencia en pacientes a quienes se les realiza irrigación del tubo a tórax vs a los que no se les realiza irrigación. En el presente estudio tenemos que el hemotórax retenido es una complicación del hemoneumotórax traumático que produce infección de la cavidad pleural (piotórax, empiema) y/o fibrotórax que requieren reingresos hospitalarios para administración de antibióticos, drenaje y decorticación por videotoracoscopia (VATS: Toracoscopia asistida por video) o toracotomía. Esto genera hospitalización prolongada y mayores costos en salud. Una opción para prevenir los hemotórax retenidos es el uso de fibrinolíticos pero en Colombia su uso no tiene registro invima y tiene efectos secundarios como sangrado. Este estudio tuvo como Objetivo: Estimar la incidencia de hemotórax coagulado y su disminución en pacientes con hemotórax o hemoneumotórax traumático a quienes se les realizó irrigación del tubo a tórax comparado con los que no recibieron irrigación en dos instituciones de alta complejidad en Bogotá entre noviembre del 2019 - marzo del 2020. Metodología: Se llevó a cabo un estudio tipo ensayo clínico con recolección de información de manera prospectiva sobre los pacientes incluidos en la cohorte, en el Hospital Universitario Clínica San Rafael y Hospital Occidente de Kennedy. Resultados: Se recolectaron durante el periodo de estudio 27 (100%) pacientes que cumplieron con los criterios de inclusión, de los cuales 3 (11%) se les realizo irrigación con solución salina para el manejo del hemotórax y 24 (89%) a quien no se realizó el manejo con irrigación. Se observó una distribución de la edad en el grupo de irrigación con una media de 51 (IQR= 22) y en el grupo que no se realizó la irrigación con solución salina con una media 37 (IQR= 22). Conclusión: La irrigación de la cavidad torácica con solución salida en hemotórax puede disminuir la incidencia de hemotórax retenido.
Introduction: The incidence of retained hemothhorax is 2- 20% (4.9) in patients with hemoneumotórax after trauma in statistics outside Colombia, however there are insufficient studies to compare this incidence in patients who are subjected to tube-to-chest irrigation vs. those who are not irrigated. In this study we have that the retained hemothhorax is a complication of traumatic hemoneumothyrax that causes infection of the pleural cavity (piotoprax, empyyma) and/or fibrotorax that require hospital re-entry for antibiotic administration, drainage and decortication by videotortion (VATS: video-assisted thoracoscopy) or toracotomy. This leads to prolonged hospitalization and higher health costs. One option to prevent retained hemothhorax is the use of fibrinolytics but in Colombia its use has no unsafe record and has side effects such as bleeding. This study was objective: To estimate the incidence of coagulated hemothhorax and its decrease in patients with hemothhorax or traumatic hemoneumotórax who were given to be irrigated from the tube to chest compared to those who did not receive irrigation in two highly complex institutions in Bogota between November 2019 - March 2020. Methodology: A clinical trial-type study was conducted with prospective collection of information on the patients included in the cohort, at the San Rafael Clinical University Hospital and Kennedy Western Hospital. Results: Collected during study period 27 (100%) patients who met the inclusion criteria, of which 3 (11%) irrigated with saline solution for the management of hemothhorax and 24 (89%) who was not handled with irrigation. An age distribution was observed in the irrigation group with an average of 51 (IQR-22) and in the group that no saline irrigation was performed on an average of 37 (IQR-22). Conclusion: Irrigation of the chest cavity with hemothhorax output solution may decrease the incidence of retained hetothhorax.
