Abstract
Los sitios de práctica son considerados como espacios de aprendizaje que permiten la articulación de los saberes, tanto disciplinares como prácticos que los estudiantes han adquirido durante el proceso de aprendizaje basados en un plan de estudios. Sin embargo en profesiones del área de la salud como es el caso de la Instrumentación Quirúrgica, se ha observado que en el desarrollo de esas prácticas en las que participan actores como docentes de seguimiento, instrumentadores quirúrgicos y estudiantes; se presenta una dificultad para un relacionamiento entre sí, lo que obliga a convertir esos espacios en lugares flexibles y contextualizados, donde se pueda caracterizar a cada uno de los actores, definir sus roles y sus alcances y así el aprendizaje de cuenta real del ejercicio profesional como una experiencia de formación. Es por esto, por lo que el objetivo de esta investigación fue analizar cómo es la articulación entre los saberes disciplinares y los saberes prácticos de los estudiantes de V semestre de instrumentación Quirúrgica de la Universidad de Santander Bogotá en la práctica profesional. El paradigma se desarrolló desde la perspectiva hermenéutica, ya que este permite estudiar los fenómenos humanos que sean significativos, bajo una mirada subjetiva con características propias de la realidad y desde los marcos de referencia que tiene el investigador que es desde donde se hace la investigación. Se plantea un trabajo con enfoque cualitativo ya que permite captar la realidad social por medio de la mirada de quienes participan en el estudio (Bonilla, 2013), el método está basado en el estudio de caso único intrínseco, apoyado en los conceptos y postura de Stake (1994), quien dice que este método estudia los casos a profundidad al igual que sus actividades, con límites claros, con identidad , intencionalidad y personalidad, para luego poder evaluar su desarrollo; por lo tanto le permitió al investigador observar las características de un grupo específico, en este caso los estudiantes de V semestre, durante el desarrollo de la práctica profesional; y analizar de manera profunda los distintos aspectos de este fenómeno, comprendiendo y evidenciando las características del mismo. Los instrumentos que se utilizaron para la recolección de los datos fueron las guías de entrevista y audios que permitieron captar el sentir de los actores participantes; las matrices en donde se registraron las categorías en coherencia con las preguntas de investigación y que permitieron precisar con seguridad la realidad y percepciones que se originaron en los registros; el formato de «récord de cirugía» los cuales permitieron clasificar las cirugías e identificar a qué tipo de estos procedimientos entraba el estudiante de V semestre. De igual manera se analizaron documentos como la «malla curricular», la cual permitió verificar si los procedimientos quirúrgicos asignados a los estudiantes correspondían o no a su nivel de práctica; el «anexo técnico» que recopila la información exigida por el Ministerio de Educación a los sitios de práctica y que evidencia el plan general de las prácticas formativas donde están específicas las competencias a desarrollar por el estudiante en cada nivel de práctica; y por último se estudia la «lista de chequeo», instrumento que le permitió al investigador tener acceso al contexto real y a la información de los sujetos en estudio de forma natural. Como resultado de este trabajo se logró comprender cómo era la articulación de los saberes disciplinares y prácticos durante el desarrollo de la práctica; y de qué manera los actores implicados aportaron en el proceso de aprendizaje de los estudiantes; así mismo se pudo identificar un sinnúmero de situaciones que involucran al estudiante y que de manera directa o indirecta le afectan su práctica profesional. Se concluyó: (a) que se deben implementar estrategias de enseñanza acorde a los nuevos modelos pedagógicos para que le generen al estudiante un conocimiento propio frente a las nuevas experiencias adquiridas; (b) que tanto el docente de seguimiento, como el instrumentador deben ser profesionales formados en el campo de la Educación, de esta manera se le puede garantizar al estudiante un apropiado desarrollo en la práctica profesional, lo que le permitirá formar un sentido de responsabilidad; (c) que existen falencias en algunos conceptos básicos en especialidades quirúrgicas como la cirugía ortopédica y de oftalmología, que se deben fortalecer desde la academia para poder garantizar al estudiante el desarrollo de las habilidades requeridas en este componente práctico; (d) que se deben definir los conceptos y elementos que componen la práctica profesional para que permitan un proceso de retroalimentación y de reflexión donde el estudiante logre articular los presaberes en el contexto real; (e) que las instituciones de salud deben propender por que los estudiantes que realizan las prácticas profesionales desarrollen las habilidades y competencias por medio de la asignación de actividades propias de su nivel de práctica. Complementando la investigación se confirmó que para los estudiantes es muy importante lograr una interacción dinámica entre los tres tipos de saberes, pero sobre todo en el saber «ser»; así mismo la universidad deberá fortalecer el concepto relacionado con el «hacer profesional», realizando prácticas reflexivas soportadas en acciones de comunicación y que se encuentren inmersas en el proceso de enseñanza-aprendizaje.
Practice sites are considered as learning spaces that allow the articulation of knowledge, both disciplinary and practical, that students have acquired during the learning process based on a curriculum. However, in health professions such as Surgical Instrumentation, it has been observed that in the development of these practices in which actors such as follow-up teachers, surgical instrumenters and students participate, there is a difficulty in relating to each other, which makes it necessary to convert these spaces into flexible and contextualised places where each of the actors can be characterised, their roles and their scope defined and thus learning to take real account of the professional practice as a training experience. For this reason, the objective of this research was to analyse the articulation between the disciplinary knowledge and the practical knowledge of the students of the 5th semester of Surgical Instrumentation at the University of Santander-Bogotá in their professional practice.
The paradigm was developed from a hermeneutic perspective, since it allows the study of significant human phenomena under a subjective viewpoint with characteristics that are specific to reality and from the reference frameworks of the researcher from whom the research is carried out.
A work with a qualitative approach is proposed, since it allows us to capture the social reality through the eyes of those who participate in the study (Bonilla, 2013). The method is based on the intrinsic single case study, supported by the concepts and position of Stake (1994) who says that this method studies the cases in depth as well as their activities, with clear limits, with identity, intentionality and personality, in order to later evaluate their development; therefore, it allowed the researcher to observe the characteristics of a specific group, in this case the 5th semester students, during the development of the professional practice; and to analyse in a deep way the different aspects of this phenomenon, understanding and evidencing its characteristics.
The instruments used to collect the data were the interview guides and audios that allowed to capture the feeling of the participating actors; the matrices where the categories were recorded in coherence with the research questions and that allowed to specify with certainty the reality and perceptions that originated in the records; the format of "surgery record" which allowed to classify the surgeries and to identify what type of these procedures the 5th semester student entered.
Similarly, documents were analysed such as the "curriculum grid", which made it possible to verify whether or not the surgical procedures assigned to the students corresponded to their level of practice; the "technical appendix", which compiles the information required by the Ministry of Education for the practice sites and which provides evidence of the general plan for the training practices where the competencies to be developed by the student at each level of practice are specific; and finally, the "checklist", an instrument that allowed the researcher to have access to the real context and the information on the subjects under study in a natural way.
As a result of this work it was possible to understand how the articulation of disciplinary and practical knowledge was during the development of the practice; and how the actors involved contributed to the students' learning process. It was also possible to identify a number of situations involving students that directly or indirectly affect their professional practice. It was concluded: (a) that teaching strategies should be implemented in accordance with the new pedagogical models so that they generate students' own knowledge in the face of the new experiences acquired; (b) that both the monitoring teacher and the instrument maker should be professionals trained in the field of education, so that the student can be guaranteed an appropriate development in professional practice, which will allow them to develop a sense of responsibility; (c) that there are shortcomings in some basic concepts in surgical specialties such as orthopaedic surgery and ophthalmology, which must be strengthened from the academy in order to ensure the student the development of the skills required in this practical component; (d) that the concepts and elements that make up professional practice should be defined so that they allow for a process of feedback and reflection in which the student manages to articulate the pre-knowledge in the real context; (e) that health institutions must ensure that students who carry out professional practice to develop the skills and competences by assigning activities appropriate to their level of practice.
Complementing the research, it was confirmed that it is very important for students to achieve a dynamic interaction between the three types of knowledge, but above all in the knowledge of "being"; likewise, the university should strengthen the concept related to "doing professional", carrying out reflective practices supported by communication actions and which are immersed in the teaching-learning process.
