Abstract
Introducción: La mejor opción para prevenir eventos tromboembólicos es el uso adecuado, racional y continuo de terapias profilácticas. Los métodos e indicaciones varían con la susceptibilidad personal del paciente a estos eventos, especialmente en el contexto de procedimientos quirúrgicos. El uso de puntajes como el de Caprina con indicaciones de algoritmos disponibles puede orientar a los médicos hacia medidas de trombo profilaxis eficaces que pueden prevenir complicaciones. Este no es el caso en muchos hospitales por lo que la evaluación de la adherencia a las guías es fundamental para todos los servicios médico-quirúrgicos Materiales y métodos: Se realizó un estudio observacional descriptivo transversal, con un análisis descriptivo para determinar la relación y frecuencia de variables. Usamos tablas de contingencia, pruebas de Fischer. Resultados: Se incluyeron 105 pacientes para el estudio. En el grupo de bajo riesgo (12 pacientes) el 100% tuvieron indicación de deambulación temprana. En el riesgo moderado (15 pacientes) solo el 33% recibieron trombo profilaxis intrahospitalaria; en el grupo de alto riesgo solo 6% de los pacientes recibieron trombo profilaxis post hospitalización, mientras que 46% recibieron únicamente intrahospitalaria, el resto no recibieron heparina. En el grupo de muy alto riesgo el 13% de los pacientes recibieron 4 semanas de heparina post hospitalización, correspondientes a procedimientos de artroplastia, el resto recibieron esquemas distintos al indicado por la guía. Conclusión: Existe una clara subestimación del riesgo de trombo embolismo venoso en los pacientes estudiados, lo que conduce a una prescripción inadecuada de medidas para reducir este riesgo, específicamente el uso y el momento de administración de heparina. Esto deberá evaluarse en la nuestra y en instituciones similares para implementar programas y apoyo para los médicos que conduzcan a una mejor adherencia a las pautas y posiblemente a los resultados de los pacientes
Introduction: The best option to prevent thromboembolic events is the adequate, rational and continuous use of prophylaxis therapies. The methods and indications vary with patients personal susceptibility to these events, especially in the setting of surgical procedures. Using scores such as Caprini with algorithm indications made available can guide clinicians towards effective thromboprophylaxis measures that can prevent complications. This is not the case in many hospitals which is why the evaluation of adherence to guidelines is essential for all medical/surgical services Materials and methods: An observational cross-sectional descriptive study was performed, with a descriptive analysis to determine the relationship and frequency of variables. We used contingency tables, Fischer tests. Results: 105 patients were included for study. In the low-risk group (12 patients) 100% had an indication for early ambulation. In moderate risk (15 patients), only 33% received in-hospital thromboprophylaxis; In the high-risk group, only 6% of the patients received post-hospitalization thromboprophylaxis, while 46% received only in-hospital, the rest did not receive heparin. In the very high-risk group, 13% of the patients received 4 weeks of post-hospitalization heparin, corresponding to arthroplasty procedures, the rest received schemes other than those indicated by the guidelines. Conclusion: There is a clear underestimation of venous thromboembolism risk in the patients studied, leading to inadequate prescription of measures to reduce this risk, specifically use and timing of heparin. This will need to be assessed in ours and similar institutions in order to implement programs and support for clinicians leading to better adherence to guidelines and possibly patient outcomes
