dc.description.abstract | Determinar la tasa de erradicación de infección por H. pylori en pacientes con enfermedad ácido-péptica del Hospital Militar Central con terapia triple estándar (Claritromicina-Amoxicilina- Omeprazol). Metodología: Estudio descriptivo retrospectivo en pacientes adultos con enfermedad acido-péptica atendidos en Gastroenterología del Hospital Militar Central tratados con terapia triple durante enero – junio 2015. A partir de los registros de historias clínicas, endoscopia de vía digestiva superior e histopatología se seleccionaron los pacientes con enfermedad ácido–péptica e infección por H. pylori con histopatología post-tratamiento y, a través del instrumento, se recopilaron variables edad, sexo, diagnóstico, comorbilidades, tabaquismo, efectos adversos, erradicación de H. pylori. Se tabularon los datos en Excel, se analizaron en SPSS versión 20. Resultados: 699 pacientes elegibles, se descartaron 568, 131 ingresaron al estudio. 50% de los pacientes es menor de 47 años, edad máxima de 88 años. 38.9% (n: 51) son mujeres y 61.1% (n: 80) son hombres. El diagnóstico más frecuente fue dispepsia 55% (n: 73), siguiendo gastritis crónica 16,8% (n: 22), reflujo gastroesofágico y enfermedad acido-péptica 11,5% (n: 15), disfagia 4,6% (n: 6). La mayoría de pacientes no presentaban comorbilidad 48,9% (n: 64), las comorbilidades encontradas fueron hipotiroidismo 9,2% (n: 12), obesidad 3,1% ( n: 4), hipertensión arterial 7,6% (n: 10), asma 1,5% (n: 2), bronquitis crónica, deterioro cognitivo 0,8% (n:1), enfermedad coronaria 0,8% (n:1), enfermedad diverticular y lumbalgia 4,6% (n: 6), adenocarcinoma gástrico, cistitis a repetición, falla cardiaca, miomatosis uterina cada una 1,5% (n: 2), artrosis lumbar, fibromialgia, hernia discal, síndrome de intestino irritable y depresión cada una 2,3% (n: 3). El 12% de pacientes referían tabaquismo (n:15). El tratamiento se administró por 10 días en 60% (n: 79). El cumplimiento del tratamiento fue 99%. 45% (n: 57) de los pacientes presentaron efectos adversos, 30% fueron gastrointestinales, 9,2% cefalea, 2,3% alteraciones del gusto. Los pacientes tratados 7 días, 38,5% presentan efectos adversos versus 49% de los tratados10 días. La erradicación de H. pylori fue 60%. Discusión: La tasa de erradicación de Helicobacter pylori en este estudio es baja, sin embargo concuerda con datos de estudios nacionales realizados hasta el momento al respecto; encontrándose tasas de erradicación alrededor del 60%, convirtiéndose este tratamiento en ineficiente con una relación costo beneficio baja; por tanto se debe considerar modificar las guías de manejo institucionales y definir nuevos esquemas de tratamiento ya recomendados por la Sociedad Colombiana de Gastroenterología; además encontramos que el 45% de los pacientes presentaron efectos adversos con predominio gastrointestinal que si bien son tolerados por lo pacientes pueden llegar a influenciar en la no adherencia a la terapia. Un meta-análisis que evaluó la incidencia de efectos adversos con quinolonas, identificó una menor frecuencia de estos con esa terapia en comparación con el grupo control (29), lo que nos lleva a considerar las terapias basadas en quinolonas como alternativa de terapia de primera línea; sin embargo, esta conducta debe basarse en el marco de nuevos estudios que evalúen la eficacia de dichos fármacos en nuestra institución para definir si verdaderamente es las más adecuadas en ésta población. | spa |

dc.description.abstractenglish | To determine the eradication rate of H. pylori infection in patients with peptic acid disease of Hospital Militar Central with standard triple therapy (amoxicillin-clarithromycin Omeprazole). Methodology: Retrospective descriptive study in adult patients with acid-peptic disease treated at the Gastroenterology services of the Hospital Militar Central treated with triple therapy during January to June 2015. From the records of medicales histories, superior digestive tract endoscopy and histopathology patients were selected acid-peptic disease and infection by H. pylori with histopathology and post-treatment, through the instrument, were collected variables age, sex, diagnosis, comorbidities, smoking, adverse effects, eradication of H. pylori. Data were tabulated in Excel, were analyzed in SPSS version 20. Results: 699 eligible patients, 568 were discarded, 131 entered the study. 50% of patients are under 47, maximum age of 88 years. 38.9% (n = 51) were women and 61.1% (n = 80) are men. The most frequent diagnosis was dyspepsia 55% (n = 73), followed by chronic gastritis 16.8% (n = 22), gastroesophageal reflux disease and peptic acid 11.5% (n = 15), dysphagia 4.6% (n = 6). Most patients had no comorbidity 48.9% (n = 64), comorbidities were found hypothyroidism 9.2% (n = 12), obesity 3.1% (n = 4), hypertension 7.6% ( n: 10), asthma 1.5% (n = 2), chronic bronchitis, cognitive impairment 0.8% (n: 1), 0.8% heart disease (n = 1), diverticular disease and lumbago 4.6 % (n = 6), gastric adenocarcinoma, recurrent cystitis, heart failure, uterine fibroids each 1.5% (n = 2), lumbar osteoarthritis, fibromyalgia, disc hernia, irritable bowel syndrome and depression each 2.3 % (n = 3). 12% of patients reported smoking (n = 15). The triconjugate treatment was administered for 10 days in 60% (n = 79). Treatment compliance was 99%. 45% (n = 57) of patients had adverse effects, 30% were gastrointestinal, headache 9.2%, 2.3% taste disturbances. Patients treated 7 days, 38.5% have adverse effects versus 49% of treated 10 days. The eradication of H. pylori was 60%. Discussion: The rate of eradication of Helicobacter pylori in this study is low, but consistent with data from national studies conducted so far in this regard; finding eradication rates around 60%, making this treatment ineffective with a low cost benefit ratio; therefore should be considered institutional change management guidelines and define new treatment regimens and recommended by the Colombian Society of Gastroenterology; also we found that 45% of patients experienced adverse gastrointestinal effects predominantly that while they are tolerated by the patients may influence non-adherence to therapy. A meta-analysis evaluated the incidence of adverse effects with quinolones, he identified a lower frequency of such therapy compared with control (29) group, which brings us to quinolones based therapies as an alternative therapy first line; however, this behavior should be based on the framework of new studies evaluating the efficacy of these drugs in our institution to determine whether it truly is the most appropriate in this population. | eng |
