Abstract
La actual situación en la prestación de servicio de los servicios de sistema general de seguridad social en salud (SGSSS), se muestra ineficiente gracias a la corrupción generalizada, desigualdad social económica y política, lo peor es que se ubica la prestación del servicio de salud como un negocio que genera ingresos y ganancias para algunos particulares.Esta problemática resulta como consecuencia de las malas decisiones administrativas que han sido aplicadas en el sistema. Para entenderlo es necesario realizar un recorrido por los antecedentes del sistema y las reformas que ha sufrido el mismo. Debemos recordar entonces que históricamente existen 3 periodos en los cuales se pueden apreciar los cambios más importantes que ha presentado la prestación del servicio de salud. El primero se inicia con la promulgación de la Constitución de 1886 hasta mediados de 1950 y en donde prevaleció lo que se puede denominar como el “modelo higienista.El sistema de salud lleva varios años en crisis. Repetidos intentos para ponerle orden han resultado infructuosos. Es evidente que el sistema tiene muchos aseguradores que no merecen ese nombre. Pocos están protegiendo a sus afiliados con ética y responsabilidad. Pero de ellos no se aprende.
Las soluciones que se han puesto sobre la mesa parecen ofrecer un salto al vacío, con incalculables consecuencias para la protección de la salud de las familias colombianas. Médicos y hospitales independientes deben reflexionar sobre las implicaciones de los sistemas públicos centralizados, de los cuales todos los países del mundo se han apartado por insostenibles, malos pagadores y por los racionamientos de servicios a la población.No podemos negar que el congreso de la república ha pretendido redefinir el SGSSS a través de ley; estableciendo principios del sistema, en el marco a partir del cual se regulan beneficios en salud, la operación, gestión y administración de la prestación de los servicios, el manejo unificado de los recursos en salud a través de la creación de una unidad de gestión financieraSin embargo; la prestación del servicio continúa generando descontento en la población y las cifras de muertos van en crecimiento; la extensas colas para adquirir una cita o lograr ser atendidos por un médico son de las principales quejas de los usuarios.

But they do not learn.The solutions that have been put on the table seem to offer a leap, with incalculable consequences for the health protection of Colombian families. Independent physicians and hospitals must reflect on the implications of centralized public systems, which all countries of the world have departed unsustainable, bad payers and rationing of services to the population.We can not deny that the Congress has sought to redefine the SGSSS by law; establishing principles of the system, within the framework from which health benefits are regulated, operation, management and administration of the provision of services, the unified management of health resources through the creation of a financial management unit But nevertheless; the service continues to generate discontent among the population and the numbers of dead are growing; the huge queues to acquire or obtain an appointment to be seen by a doctor are the main complaints of users.
