Abstract
La enfermedad coronaria va de la mano de la disfunción sexual , ambos relacionados con la oclusión de las arterias que irrigan cada uno de estos órganos ( pene y corazón ) , los factores de riesgo en ambos son compartidos , entre otros malos hábitos de alimentación , sedentarismo , presencia de diabetes así como malos controles de glicemia ( niveles de azúcar en sangre ) y tiempo prolongado de evolución , sedentarismo , tabaquismo ( uso del cigarrillo donde es muy marcada la influencia del tiempo de uso , la frecuencia y la cantidad consumida ) ,en un tiempo prolongado se genera un proceso de arteriosclerosis sistémica ( en todo el organismo ) que afecta tanto el pene como el corazón , generando con el tiempo enfermedad coronaria y disfunción eréctil ,la cual una vez que se ha presentado un evento coronario agudo se manifiesta de manera superlativa entre otras por temor a reiniciar la actividad sexual ante un posible riesgo de muerte o de repetición del evento coronario , unido esto a que para la gran mayoría de nuestros pacientes es por decirlo de alguna manera un tema tabú o un tema vergonzoso para ventilarlo con alguien diferente a su pareja .
El juego es una actividad libre sin intervención externa que evoca la libertad de jugar como tal , mezcla experiencias antiguas con las actuales , genera el hecho de disminuir si no de retirar bloqueos psicológicos que tenemos inveterados desde tiempo atrás , con el animo de que mientras se va evocando y generando alegría y placer , a su vez se van erigiendo fenómenos anticipatorios que funcionan no solo en el niño , sino también en el adulto , logrando una reelaboración placentera de sus conflictos ,desarrollando nuevos procesos de aprendizaje y encontrando otras maneras de imaginar , anticipar y resolver mediante el conjunto de procesos racionales diferentes los conflictos venideros lo que se logra por demás mediante la distracción al restar importancia a sus propios errores o fracasos favoreciendo la tolerancia a la frustración.
Lo anterior permite que ante un grupo de pacientes donde la gran mayoría son mayores a 50 años con vivencias , experiencias y familias con sus propias relaciones interpersonales preestablecidas de tiempo atrás que súbitamente vieron que todo lo que habían edificado a lo largo de su vida se podía perder por un evento coronario agudo ( súbito ) generando un evento traumático en el paciente lo que deriva en una importante barrera para entre otras muchas cosas personales y propias del paciente afectar su vida sexual con la presencia o empeoramiento de la disfunción eréctil . Tal planteamiento requiere de una estrategia pedagógica y didáctica que permite poder retomar esta vida sexual tan anhelada por la mayoría, por lo que se propone en este ensayo la posibilidad de usar la lúdica como medio para estimular a los pacientes el reingreso a su vida sexual de la forma menos traumática posible .
En este caso se proponen la formación de tres módulos de actividades cada uno de los cuales implica una administración de información puntual y especifica que consiste en un primer módulo de fisiopatología de la disfunción eréctil y su relación con la enfermedad coronaria , donde a manera de un juego de cabeza y cola se ubican los pacientes en parejas , ojala con su pareja estable y se reacomodan según las respuestas que se den a las preguntas elaboradas previamente con el fin de establecer información puntual y básica para que un paciente lego en el tema pueda entender que ocurre dentro de su cuerpo luego del evento coronario .
En un segundo modulo se propone a los participantes tomar diferentes roles que en este contexto serian el papel del medico , el papel del paciente y el papel del acompañante o pareja quienes tendrían que evidenciar y expresar de la forma que cada uno lo sienta o lo sufra según el caso y lo demuestre al resto del grupo , esto genera una identificación por parte de los demás pacientes iniciando un proceso de desbloqueo psicológico al entender que hay otros con sus mismas patologías o dolencias lo que permite al paciente poder expresar de forma mas abierta lo que siente en carne propia .
Un tercer modulo en el que los pacientes mediante actividades de representación de mímica y algunos carteles escritos explican las posibles formas de tratamiento dejando espacio para que el coordinador dela actividad refuerce la información dada para pacientes incluyendo algunos riesgos y contraindicaciones a la par de los múltiples beneficios de reiniciar la vida sexual después del evento coronario agudo .
Coronary disease goes hand in hand with sexual dysfunction, both related to the occlusion of the arteries that irrigate each of these organs (penis and heart), the risk factors in both are shared, among other bad eating habits, sedentary lifestyle , presence of diabetes as well as poor glycemic control (blood sugar levels) and prolonged time of evolution, sedentary lifestyle, smoking (use of cigarettes where the influence of time of use, frequency and quantity consumed is very marked), in a long time a process of systemic arteriosclerosis (throughout the body) that affects both the penis and the heart is generated, eventually generating coronary disease and erectile dysfunction, which once an acute coronary event has presented is manifested by superlative way among others because of fear of resuming sexual activity in the face of a possible risk of death or repetition of the coronary event, together with the fact that for the vast majority of our patients it is, to put it in some way, a taboo subject or a shameful subject to ventilate it with someone different from their partner.
The game is a free activity without external intervention that evokes the freedom to play as such, mixes old experiences with current ones, generates the fact of diminishing if not withdrawing psychological blocks that we have inveterated from a long time ago, with the encouragement that while it evokes and generates joy and pleasure, in turn anticipatory phenomena are built up that work not only in the child, but also in the adult, achieving a pleasant reworking of their conflicts, developing new learning processes and finding other ways of imagining, anticipate and solve through the set of different rational processes the coming conflicts what is achieved by means of distraction by reducing the importance of their own mistakes or failures, favoring tolerance to frustration.
The above allows a group of patients where the vast majority are older than 50 years with experiences and families with their own pre-established interpersonal relationships of time ago that suddenly saw that everything they had built throughout their lives could be lose due to an acute coronary event (sudden) generating a traumatic event in the patient which results in an important barrier among many other personal things of the patient to affect their sex life with the presence or worsening of erectile dysfunction. Such an approach requires a pedagogical and didactic strategy that allows us to resume this sexual life so longed for by the majority, so the possibility of using play as a means to encourage patients to re-enter their sexual life is proposed in this essay in the least traumatic way possible.
In this case, the formation of three activity modules is proposed, each of which implies a specific and specific information management that consists of a first module of physiopathology of erectile dysfunction and its relationship with coronary disease, where as a head and tail play patients are located in pairs, hopefully with their stable partner and rearrange according to the answers given to the questions previously prepared in order to establish punctual and basic information so that a patient layperson in the subject can understand what happens inside your body after the coronary event.
In a second module participants are proposed to take different roles that in this context would be the role of the doctor, the role of the patient and the role of the companion or partner who would have to evidence and express the way that each one feels or suffers. depending on the case and demonstrate it to the rest of the group, this generates an identification by the other patients, initiating a process of psychological unblocking, understanding that there are others with their same pathologies or illnesses, which allows the patient to express more openly what that he feels in his own flesh.
A third module in which patients through mime representation activities and some written posters explain the possible forms of treatment, leaving space for the activity coordinator to reinforce the information given to patients, including some risks and contraindications, along with the multiple benefits to restart sexual life after the acute coronary event.
